When a loved one passes away, several steps, which may be unfamiliar to you, must be taken. In order to guide you through the administrative formalities and planning a funeral, Désourdy Funeral Homes has compiled a list of practical resources you will need.

Administrative formalities to be completed

Learn more about your rights regarding leaves due to the death or funeral of a loved one.
Find out how to declare a death and registering it the Québec register of civil status. This process can only be carried out by a funeral director that has a valid permit.

When a loved one passes away, you must call upon the services of a funeral director to initiate the required process with the Directeur de l’état civil. You must fill out the form entitled Application for the simplified forwarding of information relative to the death. This will allow you to notify several government departments and agencies of the death in one step:
To settle the affaires of the deceased with respect to federal income tax
Commission administrative des régimes de retraite et d’assurances (CARRA)
To learn about the benefits you may receive if a loved one had a public sector pension plan
Commission de la santé et de la sécurité du travail
To receive benefits in the case that an occupational accident or illness caused the death of a loved one
To declare the death of a tutor or curator of a person of full age or a person under protective supervision
Ministère du Travail, de l’Emploi et de la Solidarité sociale
To finalize the administrative processes regarding programs, such as employment insurance, Québec Parental Insurance Plan, etc.
Régie de l’assurance-maladie du Québec
To cancel the deceased person’s Health Insurance and registration for the Public Prescription Drug Insurance Plan, if applicable.
To find out if you are eligible for benefits if a loved one contributed to the Québec Pension Plan. Our counsellors can also provide information regarding these benefits
To cancel the Social Insurance Number, Canada Pension Plan benefits and any other benefits
